Occasionally, you may discover that you’ve made a mistake in a clinical note or that the note is incomplete, and you need to correct or add to the note. But if the note has been locked — moved into history — you cannot update it; therefore, you must append the original note and describe the change in a new note.

Additional Tips
- An appended note is not edited into the original note but is permanently linked to the original note.
- Once you click Yes to append a note that’s in history, Dentrix automatically moves the appended note into history, and you can no longer edit or delete it.
