Attaching a Consent Form to a Treatment Plan Case

It’s important to document patient consent for treatment. Dentrix allows you to attach consent forms to treatment cases and then have patients sign them electronically, so you can manage your treatment consent forms electronically in Dentrix.

Additional Tips

  • When a patient signs a consent form, Dentrix locks the consent form so it cannot be edited or deleted.
  • When a case has a signed consent form attached to it, the signature icon appears on the right side of the case’s folder
  • A copy of the consent form is added to the patient’s Document Center, under the Treatment Planner Consent Forms category.
  • Dentrix comes with three default consent forms, but you can create your own consent forms, edit existing consent forms, and delete consent forms, as needed. To create, edit, or delete consent forms, in the Treatment Planner, click the Settings tab, and scroll to the Setup Consent Forms group box. Select New to create a new form; or, select the form you want to edit/delete and click Edit or Delete.
  • To select the electronic signature device you want to use to sign consent forms, click the Settings tab, scroll to the Select Electronic Signature Device drop-down menu, and select the device you want to use. To sign an electronic form, you typically use a mouse (a pointing device) or a signature pad (an ePad).
