Most dental providers create clinical notes to document their patients’ treatment. Often, specific procedures require the same notes but with varied patient responses. With Dentrix, you can pre-load the most frequent responses for a note template. Then, all a provider need do is load the template and plug in the appropriate “canned” response(s).

Additional Tips
- There are two parts to a clinical note template: the note text and the note prompts. The note text is the primary wording and information for the note. The note prompt is a question that should be answered by a patient, and the answer you select is inserted into the appropriate place in the note text.
- Before you create a new prompt, search the list of prompts in the Clinical Note Prompts dialog box. You may find an existing prompt that you can use, rather than recreating the same one.
- Dentrix offers multiple Response Types, including Confirmation Only, One Response from List, Checkbox Responses, Date, Number or Amount, Text, and the following selections: Arch, Tooth, Surfaces, Sextant, and Quadrant. Each type elicits a different kind of response.
- In the Edit Clinical Note Template dialog box, prompt names appear in the clinical note’s text between two tildes (e.g., ~[prompt name]~), to distinguish the prompt from the note’s text.
- Dentrix comes pre-loaded with clinical note prompts to help you create custom clinical notes. However, these default prompts may not reflect the way things work in your office, or there may be a prompt you want to add. You can create new prompts or edit existing ones to include the information you need to match your workflow.
- The “prompt text” is only seen by office workers who use the template to create clinical notes, and it’s not included as part of the clinical note.

- Insert the Health Issues prompt.
- Save the changes to the clinical note template.
