Creating a New Consent Form

Dentrix includes a few basic electronic consent forms (Consent for Anesthesia, Consent for Treatment, and Decline Treatment Consent). If your office needs another type of consent form, you can create it and save it in Dentrix for future use.

Additional Tips

  • You can use the Cut, Copy, and Paste buttons to manipulate the consent form’s text.
  • Use the Spell Check button to correct spelling errors within the text as needed.
  • When you click Save to save the form, nothing visually happens on the screen to indicate that the form was saved. However, when you click Close, the new form appears in the Setup Consent Forms group box (listed alphabetically with the other consent forms).

Check Your Knowledge

If you have access to Dentrix, check your knowledge with this exercise.

  1. Switch to the Dentrix demo database.
    (Skip this step if you are using the Dentrix Learning Edition software.)
  2. Create and save a new consent form titled, “Parental Consent – Sealants”.
  3. Enter the following text for the form: “As part of the attached treatment case for my minor child, I authorize the dentist to seal the grooves of the back teeth with a plastic coating to help prevent tooth decay. I understand that no anesthetic is needed for this procedure.”
  4. Include the required procedure codes on the consent form.
